Photo #1: Time for a really big smile! Pull your smile wide with your fingers to get this shot. You may need a friend to help you with this one!
Photo #2: Pull your left cheek out with your finger and smile wide once again!
Photo #3: Pull your right cheek with your finger and smile wide once more!
Photo #4: Open your mouth wide and tip your head back to take a picture of your upper mouth.
Photo #5: Open your mouth wide and tip your head downwards in order to take a picture of your bottom teeth!

In order for us to help provide you with the most accurate price for your treatment, please take two photos of your dental insurance card. This is entirely optionally and not required in submitting the form!
